Compass, preferably a wrist compass.

For your fifth item, you have several choices: chapstick, a solar blanket (these fold up small enough to fit in a pocket), Water purification tablets.

I would probably go with the chapstick as you specified forest, where shelter can probably be found and there will probably be a source of fresh water.
